Ultimaker extruder upgrade blocks with plates

These modified blocks for the Bertho extruder upgrade for the Ultimaker add spots to insert metal plates where the T-bolt adjustment screw head is. These plates add strength to keep the T-bolt head from tearing through the plastic and spinning while adjusting the spring tension.
