Dual mount plate for bowden extruder

EDIT:
There was a small offset (about 1.5mm) between the shape and the mount holes, it'll work, but now it's better

Based on this http://www.thingiverse.com/thing:26817 but I needed 3mm filament thickness, found this http://www.thingiverse.com/thing:151441 and adapted indieflow's plate to thematts' model
